Non-alcoholic fatty liver disease (NAFLD) is the accumulation of excess fat in the liver and can progress to non-alcoholic steatohepatitis (NASH), fibrosis and cirrhosis. Hepatic fat accumulation results from increased fatty acid uptake and de novo lipogenesis, controlled by SREBP1c, ChREBP and PPAR-gamma, together with reduced fatty acid oxidation and lipoprotein export. Insulin resistance, oxidative stress, ER stress, inflammatory cytokines and activation of hepatic stellate cells drive progression from simple steatosis to inflammation and fibrosis.
